PACE provides support services to more than 350 individuals, who have a variety of disabilities. We operate 7 supportive housing sites where people live independently along with a Community Outreach program that provides support services in people’s homes. PACE has been accredited with Accreditation Canada, since 2010.

Salary: $62,520.18 - $72,478.02
The Community Resource Navigator works collaboratively with Program Management, the Interdisciplinary Team, clients, caregivers, and community partners to enhance client independence, wellbeing, and successful community integration through coordinated service navigation, transition planning, community resource development, and client-centered support. Acting as a bridge between clients, caregivers, healthcare providers, community agencies, and internal teams, the Facilitator helps clients navigate complex health and social service systems while ensuring access to resources that support quality of life, independence, and achievement of personal goals.
This position is full-time (40 hours per week), Monday to Friday with a competitive salary and a comprehensive benefit package. Core work hours are 8:30am to 4:30pm. You are able to do a staggered start time one (1) hour either way of 8:30am. Work is performed across PACE locations, community settings, and client homes, with regular travel between sites. Given the nature of this role, you will be required to work some non standard hours and on call work is required.
