PACE provides support services to more than 350 individuals, who have a variety of disabilities. We operate 7 supportive housing sites where people live independently along with a Community Outreach program that provides support services in people’s homes. PACE has been accredited with Accreditation Canada, since 2010.

Salary:  $62,520.18 - $72,478.02

Position Summary

The Community Resource Navigator works collaboratively with Program Management, the Interdisciplinary Team, clients, caregivers, and community partners to enhance client independence, wellbeing, and successful community integration through coordinated service navigation, transition planning, community resource development, and client-centered support. Acting as a bridge between clients, caregivers, healthcare providers, community agencies, and internal teams, the Facilitator helps clients navigate complex health and social service systems while ensuring access to resources that support quality of life, independence, and achievement of personal goals. 

This position is full-time (40 hours per week), Monday to Friday with a competitive salary and a comprehensive benefit package.  Core work hours are 8:30am to 4:30pm.  You are able to do a staggered start time one (1) hour either way of 8:30am.  Work is performed across PACE locations, community settings, and client homes, with regular travel between sites.  Given the nature of this role, you will be required to work some non standard hours and on call work is required.  

What you will do:

Independent Living Facilitation & Client Planning (20%)

• Promote PACE's Independent Living philosophy and support client autonomy, choice, dignity, and self-determination.
• Facilitate individualized planning processes that identify goals, strengths, risks, barriers, and support needs.
• Coach clients in life skills development and self-advocacy.
• Support clients and caregivers through life transitions and changing support requirements.
• Collaborate with interdisciplinary teams to ensure coordinated and holistic service planning.
• Assist with complex client situations requiring enhanced coordination and problem-solving. 

System Navigation & Community Partnership Development (25%)

• Act as a resource regarding community services, funding programs, housing supports, caregiver resources, and government programs.
• Build and maintain effective relationships with healthcare providers, Ontario Health atHome, community organizations, social service agencies, landlords, and funding partners.
• Support clients in navigating healthcare, housing, disability, income support, and community service systems.
• Identify and develop partnerships that increase opportunities and supports available to clients.
• Advocate for clients when barriers to service access are identified. 

Transitions & Service Coordination (25%)

• Coordinate transitions between care settings, including hospital-to-home, housing, service initiation, and discharge planning.
• Ensure transition plans include appropriate supports, equipment, services, caregiver involvement, and risk mitigation strategies.
• Facilitate communication among clients, caregivers, healthcare providers, and community partners.
• Participate in care conferences, case discussions, and interdisciplinary reviews. 

Client Information, Education & Engagement (10%)

• Develop and share client-focused information and resource materials.
• Help clients understand available services, eligibility requirements, and community opportunities.
• Gather client feedback and contribute to initiatives that enhance accessibility and client experience. 

Program Quality, Intake & Evaluation (20%)

• Participate in intake reviews and service planning recommendations.
• Complete interRAI Community Health Assessments (RAI-CHA) and other approved assessments.
• Maintain accurate documentation and client records.
• Support data collection, reporting, and quality improvement initiatives.
• Participate in accreditation activities and represent PACE on community and sector committees as required. 

What you will bring:

Education

• Post-secondary education in Social Work, Social Service Work, Community Development, Health Sciences, or a related field.
• Life Skills Instructor training is considered an asset. 

Experience

• 3 to 5 years of experience in a community-based health, social service, or client support environment.
• Experience in case management, life skills instruction, RAI-CHA assessments, and community system navigation.
• Experience supporting adults with physical and complex disabilities.
• Experience coordinating transitions across care settings.
• Experience facilitating client-centered planning and decision-making.
• Experience working with interdisciplinary teams and external community partners, including Ontario Health atHome.
• Knowledge of AODA standards and Health Service Organization Accreditation Standards.
• Experience in a unionized environment is considered an asset.
